Grande Prairie Public School District policy requires that any student who is taking medication (prescription or over-the-counter) at school, either on their own or with teacher assistance, have a completed a “Duty of Care Form 361-1”. This form needs a physician’s signature for all prescription medication. Forms are available at reception.
